I recommend a slightly stiff brush (try a few to see what works best with your paper, though soft brushes probably won't be effective). I find brushes far better than cotton or blending sticks to create smooth tones, however I occasionally use both cotton and blending stick as well.

How do you blend with the brush? "Painting?" A stippling movement?

Mostly painting but occasionally stippling movement to push the graphite particles into the tiny crevices of the paper.
