Haruka and Michiru both wanted to have a kid. Raising baby Hotaru was fun, and then she grew up so fast -- as in, toddler to teenager overnight. Most of their friends sorta assumed Michiru would carry the pregnancy, but when the couple actually sat down and talked it over, they decided this was how they wanted it.

And Haruka has no regrets! Except when it interferes with her superpower of making straight women question their sexuality.

Feel free to make up your own headcanon for how the pregnancy came to be. Me, I've always hoped someone in Crystal Tokyo would come up with a way to make Magical Lesbian Babies (and Magical Gay Babies) for any couple who wants them. Whatever the case, "Sailor Chibi-Neptune" is just a nickname -- Michiru could look in the Deep Aqua Mirror and find out what senshi the baby will be, but they both agreed to let it be a surprise.
