Art inspired by the Junko Mizuno's Pure Trance series.This is Keiko, the sadistic director of the overeating rehab facility the manga is based on.
Those syringes in her body are called 'liquid apple' which she uses to remain in an almost constant high, where she has visions of the those little liquid apple creatures.
