(Sorry for late response... 
But if you are using the old DA layout you go to the top-right of your page and hit the "edit page" button then you should find all the widgets you can add including the donation widget.
And if you use the Eclipse layout then scroll down and in between widgets on the right side of your page should be a button that says "add section" which will include the donation one.

Well it's a safeguard in case you accidentally give points away as it is essentially currency.
I mean, it's comparable to buying something you don't want or accidentally giving money to scammers and stuff like that.
I think it would be stupid to do it without a password confirmation as there are times where I could have given away points or money that I didn't want to.

Go into your settings on DeviantArt and click the "General" tab under personal, there you should be able to reset your password.
And if you have trouble remembering passwords then write it down somewhere and put it in a safe place so no one else steals it.
