Reaper? Well, he died in an explosion, Mercy tried to play God and wanted to revive him, but the process went horribly, horribly wrong and rather than bringing him back to life, he was locked in-between life and death, never dying, constantly regenerating, and always in unbearable pain. One of the side effects to the process was that he has the ability to break himself down to as small as a single cell and then regenerate back into his standard form. That's what the smoke is: his wraith form.
