The way Sarah looks is obviously appealing to the nurse as well, she is transfixed by Sarah's movements, relishing every little nuance she can see. From the way I interpret it, (erm, read too much into it!), the nurse looks like she is keeping her legs closed for modesty, but is also subconsciously trying to control her own excitement, but, that;s probably just me.
